Section 1 of 5 Assessment Authority

1. When multiple teams are involved in assessing an application, who has the authority to decide it is ready to proceed?

No one formally owns that decision. It moves when someone acts or when a deadline forces it. There is an informal understanding of who leads, but it is not documented or consistently applied. A lead team or role is identified, but the arrangement is not formally binding across all teams. Assessment authority is clearly defined, documented and consistently applied across all review teams.

2. When two assessors review the same type of application, how consistent are their decisions?

Outcomes vary significantly depending on which assessor handles it. There is general alignment but individual interpretation creates regular variation. Standards exist but are not always applied consistently, especially under workload pressure. Assessment standards are documented, applied consistently and variation is formally managed.

3. When your team issues requests for information, how clearly defined and targeted are they?

RFIs are often vague and frequently repeated across the same application. Clarity varies. Some RFIs are well-targeted, others generate further back-and-forth. Most RFIs are clear, but there is no formal standard governing how they are structured. RFIs are structured against defined standards and rarely need to be reissued.

4. When information is submitted with an application, how is sufficiency formally assessed?

It is a judgement call made at the time by whichever officer receives it. There are informal expectations but they vary by officer and application type. Guidance exists but it is not formally binding on the assessment decision. Evidence thresholds are formally defined and must be met before assessment proceeds.

5. When a delay risk is identified during assessment, how quickly is it escalated?

Escalation happens after the delay has already occurred or become visible externally. Someone escalates informally depending on relationships and who notices. Early escalation happens sometimes, but the triggers are not formally defined. Escalation triggers are formally defined and activate before statutory risk is fixed.

6. When teams disagree on how to assess or interpret an application requirement, how is it resolved?

Resolution depends on informal relationships and who has the stronger view at the time. A senior officer usually decides, but the process for reaching them is not defined. There is some structure for resolution, but it is not consistently followed. A formal resolution process with clear authority exists and is consistently applied.

7. When multiple teams must review the same application, do reviews happen concurrently or sequentially?

Reviews are purely sequential. Each team waits for the previous one to finish. Some parallel review happens informally, but it is not structured or consistent. Concurrent review is the intent, but sequencing breaks down under workload pressure. Concurrent review is formally structured and consistently applied across applications.

8. How are complex or high-risk applications differentiated from standard ones in your process?

All applications enter the same queue regardless of complexity or risk. Complex applications are sometimes handled differently, but informally and inconsistently. A triage or tiering system exists but is not consistently applied to all applications. A formal triage system assigns applications to structured pathways based on defined criteria.

9. When the same information is requested more than once across the same application, how is that tracked?

Repeat requests are not tracked. The pattern goes unnoticed until it escalates. Individual officers may notice repeat patterns, but there is no system-level tracking. Some tracking exists, but it is not used systematically to identify root causes. Repeat RFIs are tracked systematically and used to identify and address recurring assessment gaps.

10. When an application is returned after an RFI, how is it prioritised relative to new applications?

Returned applications re-enter the same queue as new ones with no formal priority. Some officers apply informal priority, but it varies depending on who is managing the queue. Priority rules exist for returned applications but are not consistently enforced. Returned applications follow a defined priority process that is consistently applied.

11. When a precedent-setting interpretation is made on an application, how is it recorded for future use?

Interpretations are not formally recorded. Institutional knowledge sits with individual officers. Some officers keep informal notes, but there is no shared system for recording decisions. A recording system exists but is not consistently used across the team. Precedent-setting interpretations are formally recorded and accessible to all assessors.

12. When workload is uneven across the team, how is allocation managed?

Allocation is ad hoc. Applications are assigned informally without reference to capacity. Manual rebalancing happens sometimes, but it depends on who notices the imbalance. Capacity is monitored but workload balancing is not consistently structured. Workload is formally monitored and allocation is actively managed against defined capacity rules.

13. When bottlenecks form in the approvals pipeline, how are they identified and addressed?

Bottlenecks are identified only after they become visible in reporting or complaints. Experienced officers notice bottlenecks informally, but there is no systematic tracking. Some reporting exists, but it does not consistently drive corrective action before delay compounds. Bottlenecks are systematically tracked and trigger defined corrective responses before delay compounds.

14. When an application is at risk of breaching its statutory timeframe, how is that managed?

Breaches are identified after they occur. The process does not generate early warning. Escalation happens close to the deadline, leaving limited time to intervene effectively. Early warning exists sometimes, but it is not formally triggered or consistently acted on. A formal early warning system activates at defined time thresholds and triggers structured response.

15. How effective is your pre-lodgement process at filtering out applications that are not ready for formal assessment?

Pre-lodgement filtering is largely ineffective. Incomplete applications routinely enter formal assessment. Some filtering occurs informally, but incomplete applications still regularly cause downstream rework. Pre-lodgement guidance exists but does not consistently prevent incomplete submissions. Pre-lodgement requirements are clearly defined and effectively reduce incomplete submissions at intake.
